Checklist Part 1 - General Information| Information | Reason |

Checklist Part 2 - The Floor| Information | Reason |
| The use to which the precast floor is to be put (i.e. domestic, games room, garage, office, storage, etc...) | To ensure floors is designed for correct loadings |
| The superimposed (live) load that is required by the Architect/ Engineer. | To ensure floors is designed for correct loadings |

Checklist Part 4 - Stairs| Information | Reason |
